Initial Paperwork |

As a new patient, you will be asked to fill out some paperwork on your first visit. Your case history will include information about your general health, family history of health problems, and information about past accidents and/or injuries. Insurance information will also need to be provided at this time.

Physical Exam |
| |
After reviewing your health history, a thorough physical examination will be performed, with special attention to your spinal health. Your body structure will be evaluated as well as your range of motion. You may be asked to bend and twist. Palpation will also be performed to check for tenderness, pain, and swelling. |

| 3. |
X-rays |
| |
X-rays are sometimes required to evaluate the structure of your spine. If x-rays are needed the doctor will discuss them with you before proceeding. |

| 4. |
Diagnosis/Treatment Plan |
| |
Based on the results of your physical exam, health history, x-rays, and any additional tests, your condition and its causes will be explained to you. An individualized treatment plan will be developed and explained to you. In cases where chiropractic care cannot help, you will be referred to another specialist. Your treatment plan will include adjusting your spine and other therapies (if needed), which can assist in decreasing pain and recovery time. |

Home Instructions |
| |
Your treatment and recovery are viewed as a “team effort” between you and the doctor. You may be asked to assist in your own treatment by using ice packs or by performing various stretching and strengthening exercises at home. These home instructions are an integral part of your recovery. |

Follow-up visit |
| |
Make sure to schedule your follow-up visits and stick to your treatment plan. Initially visits will be more frequent. As your spine returns to its normal position and function, fewer visits will be needed. In order to achieve the best results form chiropractic care you should keep your appointments and follow the doctor’s recommendations. |
